Can a US citizen go to a website like www.nine.com that is located offshore in South America and wager, make money, win, and deposit the money in US accounts? Is it a state by state issue or what?
you can only do that if the credit card you use isnt processed by a US company. The government didnt ban internet gaming just the processing of credit card transactions related to it. so if you’re sure you are getting around that I believe you are ok. My sense, from what my friends tell me, is that you will have a hard time getting around that.

No ramifications at all. If you are a US citizen you can use ePassporte or MyWebAtm fo deposit money. I hit upon http://www.online-gambling-made-easy.com and found some good info on what to expect if you want to plunge.
